✨ Position Details

We are seeking an experienced Home Health Physical Therapist (PT) for a travel contract opportunity serving patients throughout South Chicago, Illinois.

This position involves providing physical therapy services in patients' homes, including assessments, treatment planning, rehabilitation, and discharge planning.

Requirements

Minimum 2 years of Physical Therapist experience
At least 1 year of Home Health experience required
Active Illinois Physical Therapist license required
BLS certification required
Bachelor's degree or higher required
OASIS experience required
First-time travelers accepted

📋 Schedule & Additional Details

Schedule: Monday-Friday | 8:00 AM - 5:00 PM
Weekly Hours: 40
On-Call: None
Patient Visits: Approximately 6 visits/day; 4-5 when completing initial OASIS assessments
Equipment: Laptop and cell phone provided

💵 Compensation

Travel Contract:

Weekly Gross Pay: $2,677
Taxable Pay: $670/week
Non-Taxable Stipend: $2,007week
Overtime Rate: $33.50/hour

Local Contract:

W-2 Hourly Rate: $59.45/hour
Overtime Rate: $89.18/hour
