Weekend Requirement: Every 4th weekend
Call Requirement: None
Coverage Area: Van Buren, Calhoun, and Kalamazoo Counties

Weekly Pay: $2,707

Taxable Pay: $1,440/week
Non-Taxable Pay: $1,267/week

Requirements:

Home health experience required.
Active Michigan PT license required.
BLS certification (AHA) required.
Valid Michigan driver's license.
Reliable personal vehicle for daily business use and proof of auto insurance required.

Responsibilities:

Evaluate patients and develop individualized physical therapy treatment plans.
Provide home health PT services to improve mobility, strength, and functional independence.
Monitor patient progress and adjust treatment plans as needed.
Educate patients and families on rehabilitation and home exercise programs.
Maintain accurate and timely documentation in the electronic health record.
Collaborate with interdisciplinary healthcare teams to support quality patient outcomes.
