Job Requirements

• Minimum of 2 years of RN Case Manager experience in a rehabilitation or home health setting.

• Minimum of 2 years of nursing experience in a clinical setting with a frail or elderly population.

• Graduate of an accredited school of nursing.

• Unencumbered RN license.

• Current BLS certification.

• Must have reliable personal transportation.

• Bilingual in English and Spanish preferred.

Schedule and Work Environment

• 3 onsite days per week.

• 2 mobile days per week, involving home visits.

• Monday-Friday, 8:00 AM-4:30 PM.

Responsibilities

• Conduct comprehensive, in-person patient assessments and provide ongoing case management.

• Coordinate patient care, communication, and patient needs.

• Provide patient and family education.

• Attend interdisciplinary team (IDT) meetings for the assigned patient pod.

• Manage the assigned patient population and coordinate appropriate care.

• Triage patient needs and support timely clinical decision-making.

• Demonstrate strong clinical assessment skills and the ability to manage complex clinical situations.

• Collaborate effectively with patients, families, and interdisciplinary team members.

• Prioritize tasks and contribute to process improvement initiatives.

Required Skills and Experience

• Nursing knowledge and skills necessary to care for frail, elderly participants and manage complex clinical situations.

• Ability to work independently with minimal supervision.

• Strong clinical, organizational, and communication skills.

• Ability to multitask and prioritize in a fast-paced environment.

• Self-directed, highly motivated, and capable of making sound decisions during emergencies.

• Experience working with people from diverse backgrounds and cultures.

• Recent experience in comprehensive assessments, care coordination, and complex clinical decision-making.

Preferred Qualifications

• Bilingual in English and Spanish.

• Experience working with frail, elderly, or medically complex patients.

Education and Licensure

• Education: Graduate of an accredited school of nursing.

• License: Unencumbered RN license.

• Certification: Current BLS certification.

Additional Requirement: Candidates must have reliable personal transportation for home visits. Local and travel candidates are welcome.
