Position Details

Contract Duration: 13 Weeks
Schedule: Monday-Friday, 8:00 AM-5:00 PM
Hours: 40 Hours per Week
On-Call: On-call required if requested. On-call needs are subject to change based on staff and branch needs.
Productivity Units: 30 Units
Qualifications: HCHB, OASIS, and Home Health experience required.

Required Skills & Experience

Current State License as a Physical Therapist - Required.
Proof of current CPR certification - Required.
Current driver's license, reliable transportation, and insurance - Required.
1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting - Required.
Previous home care experience - Preferred.
HCHB EMR experience - Preferred.

Required Education

- Bachelor of Science in Physical Therapy from an American Physical Therapy Association (APTA)-approved program - Required.
