Position Overview

ProTouch Staffing is seeking an experienced Registered Nurse Case Manager (RN CM) for a travel assignment with Providence St. Joseph Hospital Eureka in Eureka, California.

The RN Case Manager will provide inpatient case management services, coordinate patient care, support discharge planning, and collaborate with physicians and multidisciplinary teams to ensure safe and effective transitions of care.

Requirements

Active RN license
Minimum 2 years of Case Management experience
Acute care hospital experience - REQUIRED
Strong inpatient Case Management experience preferred
First-time travelers accepted with Clinical approval.
Experience with Epic preferred/required for charting.
Strong communication, assessment, organization, and care coordination skills
Ability to work independently and as part of a multidisciplinary team

Schedule & Weekend Requirements

Shift: 8:00 AM - 4:30 PM
Days: 5×8 hours
Weekend: Every other weekend (EOW) required
Call: Not required
Floating: May float between like units based on facility needs

Charting & Facility Information

EMR: Epic
Parking: Free parking is available on the first day; assigned parking information will be provided during orientation.
Start Date: ASAP
