Overview
A home health program in the Charlotte, NC region is seeking an experienced Home Health Registered Nurse for a 13-week contract. This is a field-based role. You will manage a caseload of patients in their homes across a multi-city territory, supported by a strong team environment and dedicated clinical leadership. First-time travelers are welcome if they are seasoned in home health.
Territory and Travel
You will make visits throughout University City, Cornelius, Davidson, Kannapolis, Salisbury, Concord, Huntersville, and the cities in between.
Mileage is reimbursed weekly.
The best central housing locations are University City and/or Concord. You must be comfortable floating across the full territory.
Radius rule: The traveler's permanent address must be at least 50 miles outside the home unit's location.
No company car is provided.
Schedule
Shift: 8-hour day shift, 8:30am - 5:00pm, 40 hours/week
Weekends: No weekend rotation, unless you are willing and interested in weekend work or the position is posted as weekend-only
On call: Not required, but available if you are interested
Holidays: Travelers are expected to work holidays unless the manager approves time off
Schedule cycle: Every 13 weeks
Key Responsibilities
Complete 6 visits per day (routine visits, discharges, ROC, recerts, and reassessments)
Meet the productivity requirement of 30 visit equivalents (VEs) per week. Cancelled appointments do not count toward VEs.
Schedule patient visits the day before each planned visit
Perform comprehensive patient assessments and complete OASIS documentation
Provide case management and episode management for up to 25-30 patients at a time
Deliver disease management and patient education
Coordinate care with patients, families, and providers by phone
Document in the EMR and upload discharge instructions and key documents (such as wound photos) to the designated platform
Attend a branch clinical meeting about once a week (in person, and occasionally virtual)

Required Qualifications
Active RN license (NC-compliant)
Minimum 1 year of Home Health nursing experience
Homecare Homebase (HCHB) EMR experience. This is required, with no exceptions.
Experience with IV therapy, Pleurx, wound vac, wound care, assessment, disease teaching, OASIS, and case management
BLS certification
Valid driver's license and proof of auto insurance

Focura for uploading discharge instructions and wound photos
A company-provided Samsung Note 9 phone for documentation and for calling patients and providers
Orientation and Support
Facility NEO (new employee orientation)
5 days of orientation with a preceptor
Support from departmental leadership, a scheduler, a Clinical Manager, a Clinical Supervisor, and an experienced clinical team
