Position Overview

ProTouch Staffing is seeking an experienced Registered Nurse Case Manager (RN CM) for a travel assignment with Providence Holy Cross Medical Center in Mission Hills, California.

The RN Case Manager will coordinate patient care and develop and support discharge plans, identify barriers to discharge, and collaborate with physicians and multidisciplinary teams to ensure safe, appropriate transitions of care.

Required Qualifications

• Active RN license

• Minimum 2 years of acute care hospital Case Management experience - REQUIRED

• Hospital Case Management experience is a MUST

• Experience performing Care Coordination and Discharge Planning - REQUIRED

• Strong understanding of inpatient case management workflows

• Excellent communication and organizational skills

• Ability to collaborate effectively with physicians, nurses, social workers, and other healthcare professionals

• First-time travelers are accepted and will be screened/approved by Clinical

Important: Candidates must have 2 years of acute-care hospital Case Management experience specifically involving Care Coordination and Discharge Planning. Hospital experience is mandatory.

Schedule & Assignment Details

• Shift: 8:00 AM - 4:30 PM

• Schedule: 5×8 Days

• Start Date: ASAP

• Floating: No

• Weekend: Weekend rotation required and must meet department needs

Key Responsibilities

• Perform comprehensive case management assessments for hospitalized patients

• Coordinate patient care throughout the inpatient stay

• Develop and facilitate effective discharge plans

• Identify barriers to discharge and coordinate appropriate solutions.

• Collaborate with physicians, nursing staff, social workers, and other members of the care team

• Coordinate appropriate post-acute services and resources

• Monitor patient progress and support timely transitions of care

• Communicate with patients and families regarding discharge planning and care needs

• Maintain accurate and timely documentation

• Support quality, safety, and continuity of patient care
