Position Overview

We are seeking an experienced Physical Therapist (PT) for a Home Health assignment based in Pueblo, CO. This position involves travel across multiple counties and offers consistent weekday hours with guaranteed weekly hours.

Qualifications

Experience:
- 1-2 years as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous Home Health experience - Preferred
- HCHB (Homecare Homebase) EMR experience - Preferred
Licenses & Certifications:
- Current State License as a Physical Therapist - Required
- Current CPR certification - Required
- Valid driver's license with reliable transportation and insurance - Required
Education:
- Bachelor of Science in Physical Therapy from an APTA-approved program - Required
