Job Title: Registered Nurse - Home Health

Location: 100 Hospital Ave, Dubois, PA 15801

Specialty: Home Health
Shift: Days, 5x8-Hour (8:00 AM - 4:30 PM)
Schedule: Monday - Friday, no weekends, no call
Contract Length: 13 weeks
Start Date: 11/16/2026

Requirements & Qualifications:

Minimum 2 years of experience, with both Home Health and acute care experience preferred
Must have RN Skilled Medicare-certified home health experience (private home care experience does not qualify)
Home Health Medicare OASIS knowledge and experience required (OASIS experience strongly preferred)
First-time travelers accepted, provided experience requirements are met
BLS required
Strong technology skills, including documentation on laptop, iPad, and iPhone
MATRIX EMR experience highly preferred
No local candidates (must reside outside a 50-mile radius of the facility)
Travel pairs welcome

Patient Population:
Primary diagnoses include CHF, hypertension, diabetes, total joint replacement (TJR), and oncology/cancer.

Observed Holidays: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas
