Pay Rate + IRS Mileage Reimbursement: $0.76/mile

Job Details:

Guaranteed Work Week: 40 hours.
Coverage Area: Marin County.
May be required to assist in additional territories within the facility's service area as needed, approximately within a 60-mile radius from the facility address.
Average daily caseload: 4-6 visits per day depending on the type of visit for PT/OT/SLP.
Productivity expectation: 28-32 points per week for PT/OT/SLP.

Productivity Breakdown:

Start of Care (SOC): 2.5 points
Resumption of Care (ROC): 2 points
Evaluations/Recertification/Discharge OASIS: 1.5 points
Discipline Discharge, Routine Visit, 30-Day Reassessment: 1 point
Weekly Case Conference: 1 point

Requirements:

Home Health experience required.
OASIS experience required.
No exceptions.
