Requirements

Current Physical Therapist License in the state of Massachusetts - Required
Current CPR Certification - Required
Valid Driver's License with reliable transportation and active auto insurance - Required
1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting - Required
Previous Home Health/Home Care experience - Preferred
HCHB EMR experience - Preferred

Education

Bachelor of Science in Physical Therapy from an American Physical Therapy Association (APTA)-approved program - Required

Key Responsibilities

Provide physical therapy services to patients in their homes.
Evaluate patients and develop individualized treatment plans based on patient needs and physician orders.
Deliver therapeutic interventions to improve mobility, strength, balance, and functional independence.
Monitor patient progress and modify treatment plans as appropriate.
Complete accurate documentation of evaluations, treatments, progress notes, and outcomes.
Collaborate with physicians, nurses, caregivers, and other members of the healthcare team.
Educate patients and caregivers on exercises, mobility techniques, safety precautions, and home exercise programs.
Travel throughout assigned service areas to provide patient care.
