Allied - Physical Therapist (PT) - Home Health

Local or Travel | Reading, PA

Start Date: ASAP
Duration: 13 Weeks

Schedule:

Monday - Friday
8:00 AM - 5:00 PM
40 hours per week

Pay:

Pay Rate + IRS Mileage Reimbursement: $0.725/mile

Job Details:

Guaranteed 40-hour work week.
Coverage area includes Berks County.
Therapist may be required to assist in additional territories within the facility's service area as needed, approximately within a 60-mile radius from the facility address.
Average daily caseload is 4-6 visits depending on the type of visit for PT/OT/SLP.
Productivity expectation: 28-32 points per week for PT/OT/SLP.

Productivity Breakdown:

Start of Care (SOC): 2.5 points
Resumption of Care (ROC): 2 points
Evaluations/Recertification/Discharge OASIS: 1.5 points
Discipline Discharge, Routine Visit, 30-Day Reassessment: 1 point
Weekly Case Conference: 1 point

Requirements:

Home Health experience required.
OASIS experience required.
No exceptions.

Additional Information:

2-week cancellation policy.
Any overtime must be pre-approved and requires written approval from the Administrator.
