Required Qualifications

Active RN license in the state of practice
2+ years of Home Health experience
1-2 years of RN clinical experience
Homecare Homebase (HCHB) experience - Required
OASIS documentation experience - Required
Current CPR certification
Valid driver's license, reliable transportation, and auto insurance

Education Requirements

Associate Degree in Nursing (ADN) or Nursing Diploma - Required
Bachelor of Science in Nursing (BSN) - Preferred

Preferred Qualifications

Certified Hospice and Palliative Nurse (CHPN) certification
